Ardent Solutions - Virtual Diabetes Self-Management Program
Ardent Solutions is partnering with the Western New York Integrated Care Collaborative to provide a virtual Diabetes Self-management Program. This is a 6-week, evidence-based program where participants are taught tools to manage their blood sugar, explore physical activity options, adapt communication styles and cope with stress. Participants will also learn from one another through brainstorming, problem-solving and feedback activities.
The Diabetes Self-Management Program is scheduled for Thursday afternoons beginning October 15 – November 19, 2020; 1:00p.m. – 3:30p.m. There is no cost to participate. Funding for this program is provided through a grant from the Administration for Community Living and New York State Office for the Aging. Participants must have a computer or tablet with internet access and an email to attend, and plan to attend all 6 sessions. Participants will receive 1:1 Tech Support prior to the first session.
