Please be advised that due to my retirement I am discontinuing the practice of dentistry on Wednesday December 30, 2020. I shall not be attending to you professionally after that date. Wellsville Dental Associates will continue with Dr. Robert Reilly. All three dental hygienists will remain with Dr. Reilly and they will continue to see my former patients.
Please be advised of your need for continued care. If you are not acquainted with another Dentist and desire to find one, I suggest the 8th District Dental Society. The telephone number is (716) 995 6300.
I shall make my records of your case available to the Dentist you designate below. Since your records are confidential, I shall require your written authorization to make them available to another Dentist. For this reason, I am including at the end of this letter an authorization form. Please complete this form and return it to me.
I am sorry that I cannot continue as your Dentist as I recently turned 80 years old and it is time to stop!! I have enjoyed my time and friendships with you all and extend my best wishes for your future health and happiness.
